Empath Health is the country’s largest non-profit hospice and home care-based health care organization. Empath Home Health is currently seeking a FT Physical Therapist to see patients in Lake, Marion and Sumter Counties. This is an excellent opportunity to join a growing Home Health agency!

DUTIES AND RESPONSIBILITIES:

• Performs initial and ongoing patient assessments in response to patients’ needs and in accordance with physician orders.
• Initiates a written plan of care, re-evaluates and updates the plan as necessary.
• Provides skilled physical therapy modalities within the therapy scope of practice.
• Supervises Physical Therapy Assistants to assure patient care needs are met.
• Plans patient discharge appropriately; provides information about community resources to address patient’s ongoing needs.

QUALIFICATION REQUIREMENTS:

• Current licensure as a Registered Physical Therapist in the state of Florida
• Minimum of two (2) years experience as a Physical Therapist.
