HIV Case Worker

Successful candidate must have solid experience & advanced knowledge of:
Associate Degree in social work, human service, or related field.
6 months of experience in the field of HIV.
Basic computer and data entry skills.
Minimum of 3 years’ experience in health care and/or human service area.
Knowledge of medical terminology.
Knowledge of (socio-economic, culture and other) barriers to medical care and ability to diminish barriers on the patient’s and SCHC’s behalf.
Successful candidate must be able to perform primary functions of position:
Assist the Case Managers in working with the program patients. Provide care coordination, linking case-management with access to care to include home visits.
Facilitate patient advisory and education groups.
Educate program clients in preventive health habits including harm reduction.
Educate community partners on HIV, HCV and STI programs, testing and advocacy.
Provide HIV, HCV and STI counseling, testing, and linkage to care for assigned counties.
Assist with social networking, program promotion and outreach for assigned counties.
Assist with social media and marketing activities.
Assess patients for social service, substance misuse and mental health needs and provide assistance and referrals.
